Registration Requirements.

All patients are required to use a unique Personal Identification Number (PIN) to verify their identify. If you have not received your PIN, please visit your location of care to request one. A PIN cannot be provided over the phone. You will need to provide a valid identification to receive a PIN.

Registration Process

Once you have have received your PIN, please note that you will need to complete all 3 registration steps in order to start receiving secure messages from our office:

1: Establish a login by creating your own user ID and password.

2: Verify your identity using the PIN.

3: Submit your preferred method of contact
